What is a Viking Eligibility (Red) Card?
A Viking Eligibility Card is proof that you have satisfied all requirements necessary to participate in WIAA Athletics at Mt. Horeb High School. Viking Eligibility Cards are distributed through the high school office, beginning in August. You cannot get the cards from your coach.
How do I get my Viking Eligibility Card? 5 Required Items:
1. Turn-in a WIAA Physical or Alternate Year Card to the high school office. *Alternate Year Card/Emergency contact information should be completed via online annual update registration for the 2020-21 school year. **Link can be found by logging into your Parent Portal.
2. Submit acknowledgement of the Code of Conduct
3. Complete YEARLY Concussion Waiver; this should be completed via online annual update registration for all students.
4. Pay the Athletic Fee. The athletic fee is $75 per sport and can be paid online or in the high school office - fees will be posted upon confirmation of the season start details; please hold off on paying at this time.
WIAA Statement & Guidelines for Physicals during COVID-19: The WIAA is offering an extension of a student's physical for the 2020-21 school year for students who meet all of the below criteria only:

Student has a physical examination on file at the school within the past 2 years, but was expiring this year.

Extention is only available if the student's primary care physician is unable to provide a new physical due to inability to provide appointments for this.

Under no circumstances will this extention be valid by the WIAA beyond the 2020-21 school year.

Athletes: The Viking Eligibility Card is due to your coach by the FIRST day of practice.
