Free & Reduced Price Meal Applications 2020/2021
Paper applications will be required for the 2020/2021 school year. They may be printed off this website. Please return your completed application to Diane Beckmann by email or mail at the High School, 305 S. 8th Street, Mount Horeb, WI 53572. Applications can be filled out at any time during the school year as income changes.
Please complete the Permission to Share Information form to be considered for other programs.
Please note that applications will not be processed until August 10. You will not hear the results of your application until after that date.

Additionally, program information may be made available in languages other than English.
To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
